TRANSCRIPT REQUEST
If you graduated/attended prior and up to 1990 the procedures below should be followed:
Records Management processes transcripts for former students who last attended Miami-Dade County Public Schools in 1990 or before. Individuals requesting records must complete the Student Transcript Request form (FM-7335) and include a copy of a picture identification (regardless of whether the request is mailed, faxed or emailed).
The Records Management staff replies to all requests as quickly as possible. Requests are processed on a first-come, first served basis. Requests will be processed within 20 business days after receipt of the completed form at no charge.

If you graduated/attended after 1990 the procedures below should be followed:
Please provide the following information:
- Last name, First name (while attending high school)
- Date of birth
- Year of graduation (if not graduated, years of attendance)
- Copy of a picture identification
- Please indicate whether you will be picking up the transcript or provide the name and address ($1.00 PER TRANSCRIPT) where you want your transcripts to be mailed.
- Please provide a contact number in case of a need to verify information
